Forms

The Student Activities office is here to serve as a resource for student organizations.  The following forms will help you in planning and participating in activities at CMC.

Event Registration Form - This form must be completed by any student or group planning an event on campus.  The form must be returned to the Director of Student Activities in Heggblade Center.  For more information about hosting an event see the Event Hosting Guidelines.

McKenna Auditorium Request Form - This form must be completed and turned in with an event registration form if your event will be in McKenna Auditorium.  The form must be turned in to the Director of Student Activities.

Room Reservation Form - This form must be completed to reserve a classroom.  The form must be turned into the Director of Student Activities.

Reimbursement Request Form - This form is used for reimbursement for events funded by the Student Activities Office.  It must be completed and returned to the Director of Student Activities along with the original receipts attached. (Your activity must be pre-approved by the Director of Student Activities)
