CMC Federal Grants Proposal Review Form (PRF)
The Office of Sponsored Research requires that for each proposal submitted through the College, the principal investigator must fill out a CMC Federal Grant Proposal Review Sheet. This is a simple form that provides a very brief summary of the working proposal for reviewers, and helps to keep track of the necessary signatures and compliance certifications required for submission. In addition, the form exists to prevent multiple proposals being sent to granting organizations that will only consider one.

If you are conducting research that involves human subjects or human participation, please visit CMC's Institutional Review Board web site to review the policies and guidelines, and then visit the IRB Application Materials to download the appropriate forms.

CMC's Significant Financial Conflict of Interest (FCOI) Policy and Disclosure Form
CMC requires that all PIs and investigators included on any federal research grant file an FCOI disclosure form prior to the submission of the grant proposal.
